How to Navigate the 504 Change Agreement Process
Step-by-step guide for 504 coordinators to document and save changes to a student's accommodations through the Change Agreement workflow in K12 Plus.
A Change Agreement allows a 504 coordinator to formally amend a student's existing accommodations outside of an annual review or re-evaluation. This guide walks through the complete Change Agreement workflow — from initiating the amendment to documenting the reason for change and saving the finalized record.

504 coordinators who need to update a student's accommodations between scheduled review meetings.
Before You Begin
- You must be logged in as a coordinator.
- The student must have an active 504 plan in K12 Plus.
- Have the specific accommodation changes and effective date ready before starting.
Steps
- Log in as coordinator — Ensure you are logged in with coordinator credentials.
- Access Meeting Manager — Navigate to the 504 Plan Meeting Manager. Click Rules and select Review Meeting.
- Start 504 Review — Click Start 504 Review and select the relevant student.
- Initiate the Change Agreement — Click Change Agreement, then Start Review, then confirm with Yes.
- Verify student information — Double-check all fields in the Student Information tab for accuracy.
- Specify the purpose of amendment — Document the reason for initiating the change in the Purpose of Amendment tab.
- Edit accommodations — Click the pencil icon next to an accommodation to edit it, make changes, then click out of the field to save.
- Delete accommodations if needed — Click the trash icon and confirm with Yes.
- Update state testing accommodations — Repeat the edit or delete process for state testing accommodations if applicable.
- Set the effective date — Select the date the amendment takes effect.
- Save changes — Click OK and then Save.
- Complete the summary and reason tabs — Document what was changed and why in the Summary of Changes and Reason for Change tabs.
- Final save and exit — Click Save to finalize, then Back to exit the workspace.
Note: Double-check the effective date before saving. This date determines when the updated accommodations apply.
Troubleshooting
- Change Agreement option not visible — Confirm the student has an active 504 plan.
- Edit icon not responding — Ensure you have clicked Start Review and confirmed with Yes.
- Effective date not saving — Confirm a date has been selected before clicking Save.
