How to Log Student Accommodations in K12 Plus

Step-by-step guide for teachers to log classroom and state testing accommodation usage for students with 504 plans in K12 Plus.

Teachers are responsible for documenting when and how student accommodations are used. This guide walks teachers through logging both classroom and state testing accommodations in K12 Plus — including how to note student refusals, attach supporting documents, and submit completed accommodation logs.

Teachers and campus staff who have been assigned students with active 504 plans and need to log accommodation use in K12 Plus.

Before You Begin

  • You must be logged in with your teacher account.
  • Review the student's 504 plan before logging to ensure you are selecting the correct accommodation type.
  • Have the date and subject for the accommodation ready before starting the log entry.

Logging Classroom Accommodations

  1. Log in with your teacher account — Access K12 Plus using your teacher credentials.
  2. Navigate to Accommodations — Click the Accommodations section and select the student from the list or search by name.
  3. Add an accommodation log — Click Add Log to start a new log entry.
  4. Select classroom settings and accommodation type — Choose Classroom Settings, then select the specific accommodation type from the drop-down.
  5. Select subject and date/time — Choose the subject(s) and click the calendar icon to select the date and time.
  6. Log student refusal if applicable — Check the corresponding checkbox if the student refused the accommodation.
  7. Add notes and upload documents if needed — Optionally add a note and upload any supporting documents.
  8. Submit the log — Click Submit to save the accommodation log.

Logging State Testing Accommodations

  1. Select state testing — Select the radial button for State Testing instead of Classroom Settings.
  2. Choose test type and subject — Select Tests, then choose the test and subject(s).
  3. Complete remaining fields and submit — Follow the same steps as classroom accommodations and click Submit.

Note: Always double-check the selected student before submitting. Accommodation logs cannot be easily reassigned once submitted under the wrong student record.

Troubleshooting

  • Accommodations section not visible — Your account may not have the correct permissions. Contact your campus coordinator or admin.
  • Student's accommodation type not appearing — Confirm the student has an active 504 plan with accommodations on file.
  • Submit button not active — Ensure all required fields are filled out, including accommodation type, subject, and date/time.
