How to Conduct a Three-Year 504 Plan Re-Evaluation
Step-by-step guide for 504 coordinators to complete a three-year re-evaluation in K12 Plus, including disability determination and accommodation updates.
The three-year 504 re-evaluation is a federally required review to determine whether a student continues to qualify for 504 services. This guide walks coordinators through each tab of the re-evaluation workflow — from initiating the review meeting to documenting disability determination, updating accommodations, and saving the final draft.

Before You Begin
- You must be logged in as a coordinator.
- Confirm the student is approaching or has reached their three-year re-evaluation date.
- Gather teacher input forms, parent input data, academic performance records, and any behavioral or evaluation documentation.
Steps
- Log in as coordinator — Confirm your coordinator role before proceeding.
- Access the 504 Plan Meeting Manager — Navigate to 504 Plan from the left-side menu and select Meeting Manager.
- Start the review meeting — Click Review Meeting, then Start 504 Review. Choose the student and select Three-Year Re-Evaluation as the review type.
- Locate the re-evaluation draft — Confirm the student status is labeled 504 Three-Year Re-Eval.
- Complete all required tabs — Work through Student Information, 504 Committee, Student Identification and Background, Parent Input, Academic Performance, Behavioral and Emotional Functioning, Attendance and Health, Evaluation and Assessment Data, Disability Determination, Additional 504 Students, Required Services and Accommodations, Implementation and Monitoring, Least Restrictive Environment Statement, and Meeting Notes and Comments.
- Save the initial draft — Click Save after each tab. When all tabs are complete, click Save Initial Draft.
Note: Click Save after completing each individual tab. Unsaved changes will be lost if you navigate away.
Troubleshooting
- Student not appearing in Meeting Manager — Confirm the student has an active 504 plan and is due for a three-year re-evaluation.
- Tabs not saving — Ensure you click Save within each tab before navigating to the next.
- Disability Determination tab showing incomplete — All required fields must be completed before the determination can be saved.
