How to Conduct a Manifestation Determination Review (MDR)

A Manifestation Determination Review (MDR) is required when a student with a 504 Plan faces disciplinary action that may be related to their disability. This guide walks coordinators through the full MDR workflow — from initiating the review and scheduling the meeting, to completing the MDR questions, recording committee determination, and saving the finalized plan.
Who This Is For
504 coordinators with coordinator-level access in K12 Plus. This process requires a committee meeting with appropriate participants.
Before You Begin
- You must be logged in as a coordinator.
- Confirm the specific incident details, including date and description of the behavior.
- Identify and confirm availability of all required MDR committee members.
- Gather relevant student documentation — behavioral records, disability documentation, and the current 504 plan.
Steps
- Log in as coordinator — Access K12 Plus and log in using your coordinator credentials.
- Navigate to Meeting Manager — Select 504 Plan from the left menu, then go to Meeting Manager and click Review Meeting.
- Start the 504 Review as an MDR — Select the student and choose MDR as the review type, then click Start Review.
- Fill out the initial draft — Enter all known information about the student before scheduling the meeting.
- Schedule the meeting — Enter a meeting title, date, start and end time, location, participants, and roles. Send the meeting invite.
- Start the MDR meeting — Open the Action Item menu and select Start MDR Meeting.
- Complete the meeting tabs — Navigate through all required student information tabs.
- Fill out the Manifestation Determination Review section — Select the reason for the MDR, enter the date of the incident, and optionally add a description of the behavior.
- Answer the MDR questions — Complete all Manifestation Determination questions.
- Record committee determination — Enter the committee's determination, notes, findings, and recommendations.
- Document parent and guardian rights — Ensure parent and guardian rights information is included in the record.
- Save and review the plan summary — Click Save, then View Plan Summary to review the completed MDR record.
Note: MDR meetings have specific compliance implications. Ensure all required committee members are present and the committee determination is documented accurately before saving.
Troubleshooting
- MDR option not available — Confirm the student has an active 504 plan.
- Meeting invite not sending — Verify all participant contact information is current and required roles have been assigned.
- MDR questions section not loading — Ensure the initial draft was fully saved before accessing the MDR questions tab.
